Wichita High School East has a long history of excellence in athletics. The Wichita High School East Sports Hall of Fame was established in 2004 to recognize individuals and teams excelling in athletics while at East or after graduating. Read more and nominate someone now!

Athletic Participation Forms

KSHSAA Physical Form
Download and print the form required for participation in all sports. This form and examination must be completed annually. A physical dated after May 1 will be valid for the following school year.

Medical Consent Form – “Blue Card”
Download and print the form required for participation in all sports. This form must be completed annually for each sport.

Pay to Participate Form – “Yellow Card”
Download and print the form required for participation in all sports except bowling. This form must be completed annually and turned in with the pay to participate fee prior to the first competition. The fee is paid once per year regardless of the number of sports a student participates in.

Transportation Waiver Form
Download and print the form required for participation in all sports. This form must be completed annually for each sport.
